solution provider to help guide you through the process and share what resources are available. Harmon Home Health assigns every client with a qualified and experienced
Home Care Nurse to ensure our staff designs a care plan that coordinates your loved one’s needs to your satisfaction.
Harmon Home Health’s professional nurses also assist families in accessing various types of community services. We take a holistic approach to the care of your loved one.
Our nursing staff provides the following services:
Care Coordination/Case Management
- Facilitate family meetings to make sure your family is aware of all your care choices and options
- Educate clients and their families
- Support caregivers and assist with locating the most appropriate outreach programs
- Work closely with physicians to ensure effective treatments
- Accompany clients to medical appointments upon request
- Work closely with certified agencies (visiting nurse associations) to ensure a collaborative approach in care
Facilitate Discharges Back Home
- Coordinate with the facility’s discharge team to ensure a safe transition home
- Conduct a home visit and evaluation for a timely discharge
and preparation - Make recommendations for safety devices and service options
Private Duty Nursing
- Larger blocks of one-on-one care
- Specializing in private “End of Life” compassionate care
- Monitoring blood pressure, vital signs, symptoms
- Preventive nursing observation
- Recovery care for post-operative elective procedures
(cosmetic augmentation)
Medication Administration/Medication Prefilling
- Weekly or Bi-weekly pillbox filling and/or prefill insulin syringes
- Administering medication
- Eye drops up to 4x/day, post-surgery assistance
- Medication Management (Maintaining refills,
calling pharmacy, etc.)
Injections
- Perform injections of Insulin, Lovenox, Infertility, and many more
- Assess effectiveness of prescribed medications
Infusion Therapy
- Compliment and supplement the frequency of what is not
covered by insurance - Home IgG Therapy (Immuno Gamma Globulin)
- Hospice supplemental hydration
Harmon Home Health’s Medical Director regularly consults with our nursing staff to ensure the highest level of care is being delivered. Telephone and in-house consults are available with the Medical Director upon request (after careful screening by the Home Care Nurse Liaison).
